If someone is in your photo, a release form must be signed by them. If they are under 18, their parent or guardian’s signature is required.

YOU MUST SUBMIT A PHOTO RELEASE & ENTRY FORM.
SUBMISSIONS WILL NOT BE ACCEPTED IF THE FORMS ARE INCOMPLETE.
